Commercial Driver Medical Certification Examination
· All paper work needed for the Federal Government and State of Texas is provided by the Medical Examiner
· Waiver and Exemption forms are NOT provided by Medical Examiner. Medical Examiner can provide these forms for a fee.
· Medical Examiner Certification issued the same day*
· DOT Medical Examination: Complete medical physical examination as required by the FMCSA to promote safety of America's roadways.
· DOT Drug Testing: FMCSA regulations require drug testing drivers who carry a CDL.
We provide medical exam services for City of Houston limousine and taxicab drivers who require the Schedule M form.
Consortium
Crown Wellness Center with TeamCME are providing a consortium service for individuals to large motor carriers to comply with the Federal Alcohol and Drug Testing (FADT) requirements. We provide the government mandated scientifically valid method for equal driver random selection. Providing this service for motor carriers reduces the hassle and cost of managing their drivers.

All drivers are required to have a pre-employment drug screen to join the consortium unless they have been in a consortium within the last 30 days.

Who is affected by this rule?
- Anyone who owns or lease commercial motor vehicles
- Anyone who assigns drivers to operate commercial motor vehicles
- Federal, State and local governments
- For-Hire Motor Carriers
- Private Motor Carriers hauling and/or operating motor vehicle > 10,001 lbs.
- Civic Organizations
- Church vehicles which carry > 15 passengers
- School Bus Drivers
What to expect / bring:
- Bring any medical records if you have a systemic / mental disease (High Blood Pressure, Diabetes, Crohns, Ulcerative Colitis, Cancer, Depression, etc...
- Please bring a list of all medications your are currently prescribed
- Be ready to urinate in a cup even if you are not getting a drug test
- Bring your glasses if you wear contacts, or need them to drive
- Bring your hearing aid if required to drive
- Bring your drivers license
- Bring previous FMCSA Medical Certificate if it is a re-examination
- Bring a smile and relax! We will do everything within the law to make sure you continue driving. We understand this is your job. :-D
